SmartDiabetes aims to establish a comprehensive, value-based model for diabetes management to improve health outcomes for people living with diabetes (PwD) and reduce systemic costs.\u00a0<\/p>\n\n\n\n

The project combines the use of the SocialDiabetes<\/a> digital therapy platform with the development of a value-based reimbursement model for healthcare payers and a data-driven operating model for healthcare organisations to guide the effective adoption of the solution.\u00a0<\/p>\n\n\n\n
