SmartDiabetes aims to establish a comprehensive, value-based model for diabetes management to improve health outcomes for people living with diabetes (PwD) and reduce systemic costs.
The project combines the use of the SocialDiabetes digital therapy platform with the development of a value-based reimbursement model for healthcare payers and a data-driven operating model for healthcare organisations to guide the effective adoption of the solution.

- Collect and analyse data to generate evidence supporting acceptance, adoption, and scalability strategies (WP4 & WP5)
