SmartDiabetes – a value-based approach to diabetes care
The number of people living with diabetes is rising rapidly and is expected to reach 67 million in Europe by 2030. Diabetes is a complex condition that requires continuous self-management, placing a significant daily burden on those living with it. Combined with an ageing population, this presents even greater challenges – particularly for older adults, who often face functional and cognitive impairments and a high rate of comorbidities. In Europe one in four people over 65 live with the condition. Empowering them with the right tools and support is therefore essential to prevent diabetes-related complications, which can further impact both their well-being and the sustainability of healthcare systems.
SmartDiabetes is a project funded by the European Institute of Innovation and Technology (EIT) which aims to establish a comprehensive, value-based model for diabetes management to improve health outcomes for people living with diabetes (PwD) and reduce systemic costs.
The project combines the use of the SocialDiabetes digital therapy platform with the development of a value-based reimbursement model for healthcare payers and a data-driven operating model for healthcare organisations to guide the effective adoption of the solution.
By focusing on “value”, SmartDiabetes seeks to improve PwD’s self-management and health outcomes while also reducing the financial burden of diabetes care. The initiative brings together a consortium of leading healthcare institutions, researchers, representatives of people living wth diabetes and industry experts, working collaboratively to establish a scalable, cost-effective approach to diabetes management across Europe.onstruction
